Before completing any surveys, you must complete the consent form.

The consent form contains some information that might not apply to you if you are not visiting the lab for a behavior testing session or an MRI scan.

Once you have completed the consent form, you are invited to complete any or all surveys that apply to your dog.

Questionnaire of Dog Life History and Behavior: This survey collects information about your dog’s breed and age, your dog’s early life, how long you have had your dog, and some characteristics of your dog’s everyday behavior.

Canine Behavior and Research Questionnaire (C-BARQ): This survey examines everyday aspects of dogs’ behavior within human households. It covers topics like attachment, trainability, fear, and aggression. This survey was created by Dr. James Serpell at the University of Pennsylvania and is widely used in canine behavioral science. Dog Personality Questionnaire (DPQ): This survey examines your dog’s temperament, or personality. It was developed by Dr. Sam Gosling at the University of Texas. Click here to visit the Gosling Lab website and learn more about the DPQ survey. Approximate time commitment: 10-15 minutes.

Dog Life Experiences Survey 1: This survey collects information about dogs’ early life experiences, including stressful or frightening experiences. Time commitment: 15-25 minutes. After completing this survey, you will be provided with a second link to optionally complete an additional 10-15 minute survey about your dog’s early life experiences.

Working Dog Questionnaire: This form collects information about working skills. Approximate time commitment: 5-10 minutes.

Howling Survey: This survey collects information about situations and sounds that cause your dog to howl. Approximate time commitment: 5-10 minutes.
